Yes you can apply for new EWS certificate or renew it online and it will be made within 2-3 days and then you can download it . Have you checked whether there is time left . Try to contact the authorities then they might provide you some time. Do not upload your old EWS certificate as it has expired and it is no longer valid . So, if you submit this then your category would not be considered rather it would be changed to open category.

It's quite difficult to make  prediction as  cut off marks/rank for medical colleges depends upon various factors like  number of students that appeared in the exam, kind of marks obtained by them , toughness level of the examination etc.

But when we go through previous data then it suggests that with 349 marks your chances for Government Medical College is very low and even you may not get Government quota seat in private medical college

For example,

In neet 2019 , the lowest marks for ewa category under Gujarat state quota that got a Government quota seat in private medical college was 408 marks and it was for Zydus Medical College and Hospital, Dahod

You are applying for TFW seat that mean tution fees waiver scheme.

TFW is scheme has been implemented by government for meritorious and economical backward students for admission in to universities and University's department.

So if you are applying for TFW category so you must have your income certificate.

If it gets cancelled then you treat as a ews category student but if you didn't any income certificate then you treated as a general category student.
