Sorry to say but you have to give NEET MDS entrance exam for doing mds course. There is no direct admission for that course.
The eligibility criteria for giving that exam is :
- Candidate must be the indian citizen , OCIs , PIOs.
- Candidate must have a BDS degree which must be associated with college or university of india under MDS course regulations 2017 issued by DCI .
- Candidates must possess a permanent registration certification issued by DCI .

since mds is master in dental and md ms is something completely different it is advisabel that you change the code and correct it. if you have applied for neet or for some other exam you can check on the official site for correction window opening date and get your code changed.

As you have doubt about what surname to enter, don't worry about it. According to NEET instructions you have to follow.
As per the NEET instructions you have to enter the surname which you have in BDS or else if possible you can also do one more thing you can apply for a name change certificate for which you need to provide your marriage certificate. They'll be giving a certificate which indicates your surname change in that case you can put your new surname which is in your Aadhar card and pan card. If this process is not possible then you can go as per NEET instructions and give name which is in your BDS certificates.
