You have to study PCB in 10+2 standard then only you can appear at the entrance exams for medical courses. You need to have PCB as main subjects in 12th then only you will become eligible to pursue any medical course and will be able to apply for recruitment as doctor.

You score is good at the moment and since you jage one year, you have plenty of time to do improvement. See, the key to crack NEET is hardwork, there is no short cut. You will have to put 10-12 hours daily without any break (may be one day off in 10 days). Competition os growing every year in NEET and to beat the competition, you will have to take extra steps.
Solve as many questions as you can, previous years question paper is compulsory.
Work on your weak areas. Suppose your Physics is weak then you will have to put 1 or 2 extra hours in it. You strategy should be made and followed by you. Consistency is the key, you cannot study 15 hours one day and 2 hours next day. You will have to be very disciplined.

See CMC Vellore does not take part in AIQ counselling but it takes part in state quota counselling of Tamil Nadu.
So as we go through previous year's (2019) cut-off then it shows that in 2019 the lowest cut-off marks was 465 for CMC Vellore under state quota for General category and we saw an increase of around 25-30 marks in institutional cut-offs in 2020. So if you are targeting for NEET 2021 then your aim should not be less than 500 out of 720 marks.

Physical Chemistry is just one part of the Chemistry. I believe you are just having problems with Analytical part of it where some maths is involved. Firstly, I would suggest you to have all the concepts clear. For example Electrochemistry might become a bit tricky is concepts are not clear. Secondly, the only possible option for you is to practice. If you can try previous year papers, and keep practising, I think you can see improvements. Mind you, first clear the concepts, then go for the questions. You will be able to see the pattern in questions. Just follow what is given and what is asked.
