I have a disability certificate in orthopaedics with 40% and I have a permanent certificate of pwd issued by my district hospital , so is there any need for another certificate from those 12 medical colleges of issued by NTA
There's reservation only for benchmark disability ( means disability should be at least 40%) .NTA doesn't recognises pwd certificate by any hospital other than those prescribed by NTA under bench mark disability RPWD Act-2016 .

I have done ms orthopaedics on a permitted seat in an institution where 3 seats are recognized and 5 are permitted by mci.....so am I eligible for fellowship courses across India and if recognition is not given by mci now to institute for 5 permitted seats will my degree be recognized???

Well before you proceed to any fellowship or to practice medicine, you need to get your degree recognised by MCI and if that is not done then practicing medicine would be difficult. If permitted seats are only permitted and not yet recognised then the degree and seats need to be recognized before you proceed further for anything else.

Manipal academy of higher education admits candidates under General or Foreign / NRI category, on fulfilling the eligibility requirements. the eligibility criteria for admission is Pass in MBBS degree from an Institution / University recognized by Medical Council of India (MCI),must have completed Internship should be on or before 31.03.2020,
Candidates holding foreign degree must be registered practitioners in the Country from where they have obtained their degree and MUST obtain a temporary registration certificate from Medical Council of India (MCI).

It is good to hear that you already got a seat under ‘A’ Category. The ‘B’ Category reservations are in this manner:
In central-government funded higher education institutions, 22.5% of available seats are reserved for Scheduled Caste (SC) and Scheduled Tribe (ST) students (7.5% for STs, 15% for SCs). This reservation percentage has been raised to 49.5% by including an additional 27% reservation for OBCs.
There is a good chance for you to get in the ‘B’ Category, but please understand that the fees are higher than ‘A’ Category.
