Dear aspirant,
Proforma 1 : confirms that bot you and your guardian/parent has been living in a state for 10 years
proforma2: it is a confirmation that your guardian/parent lived in a state of domicile for 10 years.
It is better if you submit proforma 1 if you dont have one you can go with a2 form, both are not necessary one is enough.
