Parents sign should be done before you reach examination centre because they are not allowed inside examination hall so you must prepare all the documents earlier and reach examination centre earlier to avoid any risk
You should take the following things with you
NEET admit card 2021
Passport size photograph
Postcard size photograph
Valid ID proof - PAN card, Aadhaar card, Voter ID, Driving License, Passport, Ration card
Mask on face
Gloves on hand
Personal transparent water bottle
Personal small hand sanitizer (50 ml)
You should follow all the rules regulations and covid guidelines issued by NTA NEET

High School & Intermediate Passing Certificate.
Identity Proof.
Birth Certificate.
Category Certificate (If Applicable)
Passport size photograph (Six)
NEET 2020 Admit Card.
NEET 2020 Rank Card.
Provisional Allotment Letter.
So, it's better that you if you are accompanied with your parent or guardian so that they can help you if you face any problem regarding the arrangement of document at the last moment.
