It is the decision of the State of Jammu and Kashmir that it will not participate in All India Counselling and you cannot undo your domicile just because you want to participate in the Central Counselling. Canceling your domicile is cancelling your permanent resident status in Jammu and Kashmir and forgoing all the ties with the State and that is not possible with a self declaration form. When you sign a self declaration form, you are stating that you are not eligible for appearing for Counselling in Jammu and Kashmir and Ladakh and hence you are not eligible to see admission in MBBS, BDS, Ayush courses of Jammu and Kashmir and Ladakh. You are also mentioning that you do not hold the domicile of both Jammu and Kashmir and Ladakh. But when you hold the domicile of the State you cannot give a declaration that you do not hold the domicile. That is legally incorrect and you will be held for that. Legal repercussions aside, you cannot get a self declaration done if you hold the domicile of Jammu and Kashmir. as you have domicile of the J&K. you are elligible for the state quota only,you dont have any chances to participate in the all india quota seats. But NTA has sent request to J&K UT administration to participate in 15% AIQ ,perhaps from next year J&K will b included in AIQ .But there was no official information till now.

ASCOMS is a Private Medical College has a 100 seat intake. All the 100 seats are filled through State Counselling because Jammu and Kashmir doesn't participate in All India Counselling.
- 40 seats are allotted for Government Quota and the fee is Approximately Rs.4,50,000 per student per annum.
- 40 seats are allotted for Management Quota and the fee is Approximately Rs.4,50,000 per student per annum.
- 10 seats are allotted for Weaker sections and the fees is Rs.39,000 per student per annum
- 10 seats are allotted for NRIs and the fees is approximately 19565 USD.
And all these seats are filled through NEET score on the basis of merit and preference through State Counselling. In 2019, under State Quota, 489 marks secured a seat in ASCOMS under General Category. That was the cut off. Now this is 2019 data and cut offs change every year. Cut offs change every year depending on various factors such as no.of Candidates appearing for the exam, no.of Candidates qualified, top NEET score and difficulty of the exam.
