It is not mandatory but preferable, so it would be good if you mention your name and date of photo on your photographs
The page number 31 of neet 2021 information brochure says that your photographs must be taken on or after 01.01.2021 and preferably mentioning the name of the student and the date on which the photograph was clicked - post card photo and passport size photo.

Post card size photograph is a photograph having dimension smaller than that of passport size photograph. Your Post card size photograph dimension should be (4”X6”).
Specifications of documents to be uploaded:-
*your passport size photograph,(size: 10 kb to 200 kb)

NO , there will be not any type of problem if Candidates have one year gap between 11th and 12th either in NEET UG 2021 or in NEET UG counseling process .
Candidates may be asked the reason why did they skip for a year , Candidates just need to give the relevant and an appropriate answer without any hesitation.
HAVE A LOOK OVER THE ELIGIBILITY CRITERIA FOR THE SAME ;
- Candidates should have passed 12th or equivalent with a recognized board.
- Candidates should have Physics, Chemistry and Biology as main subjects in 10+2 .
- Candidates who have given exam of 12th and waiting for their results can also apply.
- Candidates having Biology as optimal subject are also eligible.
- Candidates completed 12th or equivalent from any open school are also eligible.
- Candidates belonging to General category need 50% , from PWD 45% and others reserved category need 40% in12th the exam or equivalent.

You need to choose the code 02.
This is because,
Though you are doing BSC radiology your highest qualification with all the three subjects i. e physics chemistry and Biology remains 10 + 2.
In case, if you have done PUC in place of 10 + 2 then you need to choose code 3. For example, Karnataka Board offers puc i. e Pre University course in place of 10 + 2
While, those who are doing Bsc and have all the three subject i. e physics chemistry and Biology needs to select code 5 and those who have completed BSc with at least any two of physics chemistry and Biology in their bsc need to select code 6.

Hope you are doing great as per the question asked by you as you are mentioning that you belong to O.B.C category in state but not in centre so you are advised to fill N.E.E.T application form from general as you belong to non O.B.C in central. But at the time of admission in state quota you can certainly use your state O.B.C certificate.

Yes of course you should change it because even if NTA has not mailed you regarding your blurred image of your thumb impression, this does not mean that it won't create any problem in future, it might create so you should change that image by yourself using NEET Correction window to avoid any future complication.
NEET Correction window will remain active till August 14th evening so make all the required corrections before the deadline.
You can follow the given steps to make correction in your uploaded images :-
** Visit the official site of NEET 2021and click on the button saying' Correction in NEET UG 2021 Application form'.
** Now login using his login credentials.
** Edit/delete/modify the details whichever required in NEET 2021 application form.
** After making corrections just click on 'Preview and Submit' and then preview the form if it does not require further modification then just submit it.
** Now you will get an OTP on your registered mobile number and e-mail, just enter it and your correction will be submitted.
** Now take the print out of the NEET correction slip for future references.
Images should only be uploaded in the prescribed specifications as given below :-
