I am giving NTS examination from Madhya Pradesh but I originally belong to Bihar .My caste is certified as OBC in both Bihar and Madhya Pradesh state OBC list but it is not listed under Central list for OBC , will I get OBC reservation ?

If your caste is not in the central list for OBC, chances are there you might not be eligible for the quota/reservation. For that you can show them your original OBC certificates. Or else you will just be changed into general or unreserved category.

As far as the OBC NCL cutoff percentile is concerned it remains almost the same i.e 40 th percentile which is roughly equivalent to 135-145 marks. Therefore with your current score you will clear the OBC NCL cutoff definitely , However getting government medical colleges in these states would be a dicey as the state cutoff would go around 580 itself and since you wouldn't have domicile of a particular state thus you might not get admission in that via the state qouta , and the AIQ would also be very high as compared to 500

First off I would like to mention that there is no such management quota in any of the government medical colleges of our country and NEET examination is the only gateway to get a seat in any government, private or deemed universities.
If you wish to apply in any of the medical colleges that does belong to your domicile meaning which is not under your state government you need to apply through the AIQ also known as the All India Quota done by the MCC/ DGHS which offers 15% eligibility criteria for the candidates to apply all over the country's known government medical institutions

How do I make Non creamy layer certificate? i already have an obc certificate mentioning i belong to ncl, but this certificate was made in 2018, and josaa says i need a certificate made after april 2021. is the obc certificate different from a non creamy certificate? someone please help asap

Actually during the counseling, the most recently issued category certificate is required, that's why you need to have a recently issued caste certificate. Also your category certificate is 3 years old so that won't help your case during the counseling.
Other Backward Caste are further divided into Creamy Layer(CL) and Non Creamy Layer(NCL). To put in easy words if you wish to derive benefits of the reservation then you have to be OBC -NCL as OBC-CL candidates will be treated as General category students.
In order to qualify as an OBC non-creamy layer candidate, the applicant’s parents’ annual income should be less than Rs. 8 lakhs. Also you can check if your caste comes in category of OBC on the official website -

And yes OBC certificate is different from OBC NCL certificate, unless your certificate clearly mentions that you are OBC-NCL you can't derive reservation benefits.

While NIBM, Pune makes every effort to ensure that candidates from the EWS/NC-OBC/SC/ST/PwD categories enrol in the programme in the proportions required by law, they must first meet the eligibility requirements and demonstrate a specific level of performance to be considered for admission.
This displays that NIBM Pune does have OBC reservation.

As per information available , In order to get benefit of reservation in Central institute like IITs, NITs,IIITs , your caste or community should be in the Central OBC list. If you caste is in OBC in state but doesn't include in Central OBC list, then you could not get OBC-NCL Reservation. You will be treated as General category student.

I hope that you are doing well. It is difficult to get government seat for bhms but you can get private seat in 279 marks, if you belong to general category.
Your score should be above 520 to get government seat for bhms in state quota. This cutoff is in accordance with the last year cutoff.
Although getting mbbs seat depends upon various factors like number of students appeared in the exam, difficulty level of exam, AIQ rank, category, region, state, state rank,etc. But You can try for semi government seat or private seat.
