According to last year, 476 marks may give you all india rank of around 64000-65000
Last year the cut off for BCB in AP was also about 64800.
You have chances of admission in the following colleges according to last year
*Great Eastern medical college, Srikakulam.
*GVP H and medical technology.
But the cut off keeps changing every year based on various grounds like toughness of paper, no. of students appeared for the exam, top marks, no. of seats available etc.

By seeing 2019 NIT Raipur cutoff of mechanical engineering branch for EWS category ,It seems like not easy to get it in the first 5-6 rounds (This year only 6 rounds of Josaa ) with the rank in JEE Mains, you mentioned in your question
You can look for other branches in the college or other NITs also you can go for spot round.

Firstly to do Engineering you can either pursue BTech or BE .
Under BTech you can opt for any stream of your choice like
Aeronautical Engineering.
Automobile Engineering.
Civil Engineering.
Computer Science and Engineering.
Biotechnology Engineering.
Electrical and Electronics Engineering.
You can also opt for lateral entry through diploma .
Engineering is the third major stream with 37.70 lakh students enrolled at the undergraduate level.
You can go to IITs or nits for Engineering course.
For btech
You must have passed Class XII exam from a recognised board with Physics, Chemistry and Mathematics as core subjects.
You must also he secured minimum aggregate marks of 60 per cent in the above subjects combined.
