Yes, 12th marks are required but only to be eligible for eligible while your admission is completely on the basis of your marks obtained by you in NEET examination
To eligible for almost all the medical colleges for MBBS you just need to fulfill neet eligibility criteria that is you need at least 50% in aggregate of PCB in your class 12 if you belong to general category while in AIIMS in order to be eligible for MBBS you need at least 60 % in aggregate of physics chemistry biology /biotechnology and English if you belong to general category, though admission is completely on the basis of marks obtained by you in neet
Here's how you can get admission in MBBS :-
------------------------------) Appear in Neet examination
To be eligible for neet one must be either appearing for 12th board examination or should have passed 12th board examination with physics, chemistry ,biology/biotechnology and English and must have got at least 50 % in aggregate of PCB i.e Physics, Chemistry and Biology /biotechnology in 12th examination if belongs to general category , or at least 40% in aggregate of PCB if belongs to Obc/ Sc /st category. ------------------------------) Qualify neet examination. Qualifying cut off in neet is given below
General :- 50th Percentile
OBC:- 40th Percentile
SC :-40th Percentile
ST :- 40th percentile
To be on the safer side, out of 720 marks, target at least around 580 -630 marks for state quota seats and 650 marks in neet for All india quota seats if you belong to general category.

Yes it is required almost everywhere, whether you are seeking admission through NEET UG or if it is on the basis of any other entrance examination for courses like B.Sc. Nursing or BPT or any other course.
Class 12th marks are only required to determine your eligibility for admission to medical colleges through NEET UG like if you won't be able to secure minimum qualifying aggregate in 12th as mentioned in the eligibility criteria below then even after qualifying NEET UG you won't be able to appear for counselling
NEET UG eligiblity criteria :-
>>>>> You need to pass separately in theory as well as in practical of PCB.
>>>>> If you belongs to General/UR category then your aggregate of PCB (Theory + Practical) should be equal to or more than 50%(40% for SC/ST/OBC and 45% for PwD).
