The syllabus of NEET is based on the 10+2 curriculum including Physics Chemistry and Biology.
Some of the tips you can follow for prepration of NEET are:
Follow the NCERT books of 11th & 12th std strictly and mug up all the concepts,formulas etc as more than 80% questions are directly been asked from NCERT
Look for the previous years papers of the NEET to know about the standard and difficulty level of the paper.
Stay confident and determined for your goal, figure out your weak portions and work on them.
Try to solve more and more MCQ's and mock test
Watch motivational videos and toppers interviews to boost up your confidence level

Since you are are asking for 4 month plan , I m assuming you want to complete your whole syllabus in those 4 months. So here are few tips which you can follow to achieve your goal.
1- divide the whole syllabus in 4 sections so you can devote each section to each month. And don't juggle between multiple things at once and stick to the plan
2- make sure you have all the resources sorted which you'll be need for the preparation. Here are few resources which I would suggest you to use-
-> NCERT for physics, chemistry and biology
-> DC Pandey for physics numerical practice
-> MTG fingertips for biology
-> MTG fingertips for chemistry
These are few standard books which are preferred by most of the students who are appearing for the NEET.
3- For theoretical part make NCERT your Bible. You don't need to refer anything else for biology and inorganic chemistry except for NCERT. Make sure to go through each and every line. Whereas for physics make sure u have practiced each and every solved numerical for physics NCERT.
4- Practice MCQs from above given book as much as you can. The Practice is something that can take you from bottom to top.
5- Solve mock test as much as possible. So that you can keep checking your progress from time to time. And work on your weak points.
Most importantly consistency is the key ..

See there is no any hard and fast rule of required time to prepare for such national level entrance exams like NEET or even for any other examination. But you need to get thorough with your NEET syllabus and as we know that NEET syllabus comprises of class 11th syllabus as well as class 12th syllabus so if you are starting from scratch then it is very difficult to complete whole syllabus in just 4-5 months but yes if you are in regular-touch of your NEET syllabus then it is definitely possible to complete your syllabus in 4-5 months in which you can also give 1-1.5 months for revision and solving of previous year papers and sample papers.

**** Practice previous year's question papers and sample papers as many as you can because it will enhance your self-confidence and time management skill.
