Making futuristic predictions completely accurately isnt possible when it comes to ranks and allotment since they are highly variant on the basis of number of aspirants, level of exam, etc.
However, you may still get a rough idea about the rank you might be alloted in Mahrastra state PG medical merit list, from pervious years' data.
If we go by previous year statistics, then on 690 marks, it is likely that you may get a Maharashtra state rank somewhere between 5,603 to 5,610 and All India Rank between 60,000 or so. So, a safe rank mark you may assume on this marks would be under 6,000 state rank.
Now coming to, whether or now you will be able to get a clinical subject, it seems difficult given that you are talking about general category admissions, however it is advised that you wait till the allotments gets complete.
