you must have done bsc physics from a reconginzed university or college with minimum 60 percent marks(inclusive of all subjects) those who have done bsc applied sciences are also eligible.

admission will be on basis of merit in UG.

check the official site of the institute , fill the form. if you get short listed on basis of your merit , you will be called for counselling.

admission procedure is online , go to vellore institute official site, register yourself there, fill online application form (the application  fee is non refundable) , then attach the required documents. If you get selected for counselling round , be there , and then wait for results.
