You surely can do this. Not a very difficult task assuming that you're done with almost all the concepts of biology and chemistry and only are asking for the revision pattern for the last month. See I'll guide you follow this well.

Make a habit of reading ncert chapters and reading almost every single line like that's the most important line. Because mcqs can be asked from anywhere end can be twisted in any way so more you remember things, more are the chances that you'll be able to go past a score of 320 from biology ncert itself. Keep reading one section which includes 3-5 chapters every day. For human physiology, plants physiology and genetics you can take 2 days each. Keep doing this everyday. And also keep revising what you studied previous day quickly on the next day. No need to solve mcqs now.

For inorganic chemistry, things go exactly same like biology. Just keep reading no need to solve mcqs now.

For organic chemistry, again same thing. Keep reading, keep remembering and recollecting all the reactions from Alkanes till the nitrogen containing compounds. Try to link every thing. Practice some important mcqs of possible as they'll enhance your thinking skill and boost your confidence.

For physical chemistry, let me be honest. Out of 10-12 questions that will be asked from physical chemistry, 7-9 will be numericals only. So your first goal should be revising formulas of all the numiercals chapters like solid state, solutions and colligative properties, electrochemistry, thermodynamics, etc. Just revise the formulas and solve the mcqs ok. And then go for other theorretical physical chapters like surface chemistry, general principles and isolation techniques, etc and read them as there will be 1-1 theory questions from each so that you can answer them too.

So do this regularly and honestly without even wasting a minute in a day and then surely you will be able to score 300+ in biology and 150+ in chemistry TRUST me.

If you are studying for 6 hours a day, keep swapping between subjects every hour. And make sure to take atleast 5 minutes break between every session. Also practise one mock test every week. Physics needs utmost focus and time because it involves with theoretical concepts and numbers. There should be a systematic approach in preparing for NEET. Always reducing the visual distractions: Make sure you have nothing within your vicinity to distract you. Like keep your phone a side. Or sit away from the TV, Windows where you can see people.

Try to Eliminate Noise around you: If you are someone who can concentrate with music, make sure it is light music and nothing to distract you. It should increase your ability to concentrate and not distract you. If there is any noise that might distract you, use ear plugs or a fan. Trust me, if am being distracted by noise, I switch on the fan and it works.

Self talk to control Internal Distractions: When you are noticing that the mind is drifting aside, stop what you are upto and keep a checkmark on a piece of paper along with the time. Keep a track of these checkmarks and the timings. Talk to yourself that you understand you are being distracted but you also have to study. If it keeps bothering you, write down what is it that is bothering you and promise yourself that you will come back and look into that. Now that has been parked in the notebook, get back to studying.
