Medical certificate is to be taken from registered medical practitioners as per Govt. rules

You can download the form from this link http://www.vpmmpcoe.org/Proforma-M_Medical_Fitness.pdf and get it signed by a medical practitioner (having atleast an mbbs degree). It will be accepted. All the best.

Every college has this. They need this to know that you are well enough fit to take the 4 year course. They keep those details. There will be form which will be given to you which you need to fill it from a licensed doctor. It will have normal and basic questions regarding your health. If you don't have ask to the admission department they will give you one.
