Can anyone plz suggest if i should take long term for medical keeping in mind about d changes in 2019 or go for b pharma? is b pharma good?

See it's a two edged sword and a risky thing, but if handled properly will be a cakewalk.

The standard of questions asked in NEET UG and other entrances will go up higher day by day is true in one hand, but the fact remains clear that if your basics are strong enough, they won't be of much nuisance.

So if you have the privilege to spare a year for Medical, I would HIGHLY recommend you do so.

And if things are otherwise, you can join B.pharm as it's a flourishing course too with great career opportunities.

Choice is upto you :)
