Its quite difficult to get a government seat in medical college for an OBC candidate with 486 score in NEET.As per last year's cutoff 560 and above was considered a safe score to get admission in government medical college through All India Quota. An OBC candidate must score 550 and above score in NEET to secure their seat in government medical college.

The cutoff marks changes every year. It depends upon number of candidates that appeared in examination, number of seats available, difficulties of question paper,etc. As per last year of cutoff marks with 486 marks you have no chance to get government medical seats in all India quota. You need to score at least 590 to 610 marks for getting government seats in all India quota if you belong to OBC catagory.
