Hey dear, seeing last year's cutoff which went very high that even students getting 500 marks found it hard to get a govt. Mbbs seat. Since your score is 490 so your approximated rank will be between 58040- 70580. For mbbs it would be difficult for you to get admission in all india quota. It's quite difficult to predict that whether you will get seat or not as cut off marks for medical colleges keeps changing every year depending upon various factors like number of students that appeared in the exam, kind of marks obtained by them , toughness level of the examination etc.
But when we go through previous data then it suggests that with 490 marks being from obc category you hold quite low chance to get a seat in Government Medical College. For example, in neet 2019, lowest marks required for obc category for getting a mbbs seat in government medical college under UP state quota was 573 marks and it was for Autonomous Medical College, Bahraich
But you have chances in private medical college and deemed universities.
