the categories of Disability are in JEE Main are
- Bindness and Low vision
- Deaf or Hard of Hearing
-
locomotor disability including cerebral palsy,leprosy cured,dwarfism,acid attack victim and muscular dystrophy
- autism,intellectual disability,specified learning Disability and mental illness
- multiple disability among stated above
- other specified disabilities mentioned in the Schedule of the RPwD Act 2016
candidates having 40% or more disability are eligible to apply.They must have certificate of disability
Since you meet all the requirement yes you can apply for the same

Only permanent disability card holders are eligible or temporary disability card holders are also eligible?
