• Application form print out
• Bank chalan copy
• Neet 2019 admit card
• Secondary School certificate
• 2nd puc or class 12th mark sheet
• Two passport size photos
• Study certificate attested by the Block Education Officer(BEO) or Deputy Director of Public Instructor (DDPI)

The following certificates are required for the NEET UG counselling 2019 in Karnataka State.

1. Application form printout

2. Admit card for NEEt 2019

3. Two or more passport sized photographs (recent)

4. Secondary school certificate

5. Class 12 marksheet.

6. Bank challan copy.

7. Any other certificates like the caste certificate and caste validity certificate.
