I assume that you were study class 12th and 11th very well. If not nothing to be worry. Make a schedule and follow strictly. Do revision and solve previous year question papers, solve your notes questions, Give your mock test. Revise that chapters on which have your strong grip. Don’t learn new chapters. Chemistry is scoring chapter in competitive exams. Remember all reactions and all terms. Modern Physics are scoring in physics. Maths demands practices so solve maths problems. Identify your weak portion and try to fix it. Revise, Revise and Revise only.

4. NCERT will do for organic and inorganic chemistry (along with some coaching modules if you can get them somehow)
5. This is really important: DO NOT FORGET TO DO PREVIOUS YEAR PAPERS AND SOME SAMPLE PAPERS BASED ON CURRENT FORMAT AND SYLLABUS. It helps a lot. Really.
it will be a really daunting task to finish off your preparation in 5 months alone because if you haven't had the motivation to start preparing till now, then it is going to be so tough, trust me.
Now lets plan your schedule so that you can finish all of the above in time (if you can finish all of the above books easily and master them, and also get a 90%+ score in board exams, then you, my friend are a stud. If DTU or BITS is your dream college (assuming you are from Delhi), then nothing will be able to stop you from getting an admission there).

JEE Main is one of the competitive level exam. You need to be motivated and determined to crack your exam.
In order to clear the cut off in 13 days you should work on your concepts. Take help of NCERT books.
Focus on the topics which carries more weightage in the exam.
Solve previous year questions papers and mock test series for your practice as JEE Main is all about time management.
