    Dear Sir/Mam, I got defect in my gate form with Defect type:Name Mismatched:Upload Affidavit.Basically i have uploaded aadhar card as photo id and i have same issue with collage id.Sir can you provide the format of affidavit i:e What to write in affidavit so my problem with gate form get rectified.
