No, you are not required to fill any form after clearing stage 1 of NTSE. You must begin with the NTSE Preparation 2019 for stage 2. The NTSE admit card 2019 for stage 2 will be out in the month of April on the official website of NCERT. You will be required to download it by entering your roll number of stage 1. You will be required to carry this admit card on May 12 to the examination centre.

Deat Prachi, candidates who have been shortlisted in NTSE stage 1 exam are qualified for NTSE Stage 2 exam. The stage 2 exam is a National Level Entrance Examination conducted by NCERT on May 12th, 2019. You can download the admit card from the official website during the month of April.
