Yes, you will have to fill up again but not the complete application form. You will be required to login through your application number and password (which you got after registering for Jan attempt). After logging in into the portal, you will be required to select the exams in which you wish to appear in April attempt. The final step will be to pay the required application fee.
Now since you are asking regarding the April exam I'll assume that you have already appeared on the January test and have not performed satisfactorily.
