No institute ask the students to register in their institute before appearing for NEET examination. NEET being an all India entrance exam has become a key to get admitted into medical colleges.
Only after the declaration of NEET results, you are required to choose the colleges of interest and proceed further processes.

No, you do not have to take any admission in any medical college prior to the NEET exam.
After passing the exam and declaration of the results, the next step is to seek NEET counseling. Through NEET counselling, candidates will be admitted to medical institutes across India. The seats will be assigned based on the candidate's All India Rank and their college preference.
Four rounds of counselling will be held by the MCC: AIQ Round 1, AIQ Round 2, AIQ Mop-up round, and stray vacancy round. Except for the Union Territory of J&K, candidates who qualify as part of the All India Quota would be eligible based on their NEET UG scores in 2022. Seats would be returned to the states if they remain unfilled after round 2 of NEET 2022 counselling in AIQ.
