he original certificates submitted will be retained by the Institute till the candidate successfully completes the M.D. (Hom) course duration. Candidates are required to keep sufficient nos. of attested photocopies of the documents preserved with themselves for future use/reference and record.
Nationality Certificates or Photocopy of the valid passport/ Domicile Certificate / Birth Certificate duly attested by Dean /Principal of previous institution attended.
WBPGHAT Admit Card / University selection letter.
Xth standard, admit card, pass certificate and mark sheet.
XIIth standard admit card, Pass certificate and mark sheet.
Conduct Certificate from the head of the institution last attended.
Transfer (College Leaving) Certificate from the institution last attended.
All Mark sheets of the qualifying examination.
Passing / Degree Certificate of qualifying examinations.
Internship Completion Certificate.
Attempt Certificates of all examination from head of the institution last attended.
Caste Certificate (if applicable).
OBC Caste Certificate indicating validity, issued after 31-03-2014.
Non-Creamy layer validity Certificate for DT/VJ, NT-1, NT-2, NT-3, OBC (if applicable).
Valid Registration Certificate from West Bengal Board of Homoeopathy or Central Council of Homoeopathy.
Migration Certificate issued by the respective University, (if applicable).
Self Educational GAP Affidavit by student (applicable if the GAP is more than six months after completion of qualifying degree).
Medical Fitness Certificate/Orthopaedically challenged Certificate; (The definition of “Physically challenged” will be in pursuance of the correspondence of both Medical Council of India as well as Central Council of Homoeopathy as under:
Disability between 50% to 70% i.e. locomotor disability of lower limbs vide letter No. MCI-34(1)(Gen.)/2009-Med./2569 dated 21-04-2009
Blind (including colour blind), deaf and/or dumb candidates shall not be allowed for admission in the course; vide Homoeopathy (Degree Course) B.H.M.S. Regulations (as amended up to 17-06-2005) Part – III.
Undertaking for not having taken admission to MD (Hom) Course on earlier occasions.
Four recent passport size (3.5 cm X 4.5 cm) coloured photographs and four stamp size photographs all duly attested, Polarized and computer generated photographs are not acceptable.
