You need to have the following documents while applying for JEE Advanced 2018:
These documents have to be scanne to be Uploaded :

Certificates of Class X as proof of birth or Birth Certificate

Class XII (or equivalent) examination

PIO Card / OCI Certificate

DS certificate

Gazette notification showing name change ( only by candidates whose name is not same as in Class X certificate)

Physical Disability certificate (FORM-PwD II/III/IV whichever is applicable) issued by the notified medical authority.
Dyslexic candidate must submit FORM-DYSLEXIC 1 and 2 instead of FORM-PwD issued by a doctor specializing in psychiatry and associated with the RCI/Dyslexia Associations. The certificate must mention ‘SEVERE’ so that the candidate is considered under PwD category. |

Letter issued by Chairman of respective zonal coordinating IIT (FORM-SCRIBE). |

Certificate (FORM-DS) issued by a competent authority in the Directorate of Resettlement and Rehabilitation, New Delhi under the Ministry of Defence or the Ministry of Home Affairs, Govt. of India. |
