These documents have to be scanne to be Uploaded:



Certificates of Class X as proof of birth or Birth Certificate



Class XII (or equivalent) examination

Scribe Request Letter



PIO Card / OCI Certificate



DS certificate



Gazette notification showing name change ( only by candidates whose name is not same as in Class X certificate)



You need to upload the above mentioned documents in the format and dimension of images given below:

Dyslexic candidate must submit FORM-DYSLEXIC 1 and 2 instead of FORM-PwD issued by a doctor specializing in psychiatry and associated with the RCI/Dyslexia Associations. The certificate must mention SEVERE so that the candidate is considered under PwD category.

Letter issued by Chairman of respective zonal coordinating IIT



(FORM-SCRIBE).

Certificate (FORM-DS) issued by a competent authority in the Directorate of Resettlement and Rehabilitation, New Delhi under the Ministry of Defence or the Ministry of Home Affairs, Govt. of India.

PIO card or OCI certificate, as applicable
