The Medical Certificate form is often made available from the Health University website itself, you can take a print of it and get it filled up by a registered doctor. And for the antiragging form, you can get it on the College Website or right from college itself.

Break in Study refers to whether you had a drop year in between your passing out from Class and taking admission in the college. In case of a break, you need to submit a letter to the principal concerning about the same.

Well.as far as I know medical certificate will be provided by you and anti ragging certificate will be available on the website which you need to fill and then submit the same.

Break in study means if you leave your studies in between the course of study then you need to do what they demand according to the declaration.
