It's not an issue as by scoring good marks one could just get an government seat into colleges which doesn't mean that if he has scored less marks he won't get any seat.
He may try for a seat in deemed univesities or private colleges as well for easily securing an seat.
But if the fees structure of private universities is not affordable for you, then you could simply send him abroad as in abroad for pursuing any degree into medical field the fees structure would be affordable for you.
Else he could simply drop this year and prepare well for the upcoming year and give his best in NEET 2021.

In order to join MBBS course it is mandatory to qualify NEET exam. Even if you wish to get admission through management quota, NEET minimum cutoff has to be attained. Since you have not mentioned how many marks the person you are referring to has scored, it is impossible for us to say anything for sure.
If he has not qualified NEET then either he can take a year off and give NEET next year. He can use the NEET knockout package given here.

Other he can opt for other courses like paramedical courses which does not require NEET.

Don't lose hope on securing less score in NEET. It has been aforementioned that without securing the NEET Cut off percentile, no admission will be granted in private or medical colleges.
if he has qualified NEET then, in order to take admission in a private college one has to pay a huge amount to get an admission.
Or he may drop a year and prepare again, go for other alternatives such as physiotherapist, lab technicians, radiologists etc.
