As you have mentioned that you are currently in BDS 1st year so your highest qualification with PCB is class 12th so you should use code 02.

Code 04 is used by those candidates who have completed their pre-professional/pre-medical examination with Physics, Chemistry, Biology/Biotechnology, and English after passing either the Higher Secondary examination or Pre-university or equivalent exam
