That is indeed a decent score. You won't be getting a seat in Government Colleges under All India Quota but there are chances in your state. Getting a government seat would depend upon the performance of the students domiciled in your state, availability of seats and you can also make a guess on the basis of previous years' state cut offs. Since, I'm from WB, I can tell you that if you are from WB then you will surely secure a seat in Government Colleges here under State Quota.

500 is a good score but for getting a govt college a score of 550 and above is considered safe. As you haven't mentioned your category, I am going to answer for a unreserved ones. There are less chances for a govt Medical seat. This year the cut offs may shuffle. So don't lose heart rather start studying again. Wait for results and if you think your ranking is fair enough, go for counselling.
