Most universities have really good, comprehensive websites on their specific application process. So if you want to go to a specific university, just Google them  Most likely they have all the information you need on how to apply as an international student.

You would generally need to have academic records for the last four years or so (year 9-12). As to when to apply, it differs greatly from country to country, university to university, but generally be prepared to apply before you graduate from high school.

Also, if you're planning to go to a country with a different language of instruction, then I presume you have to take language examinations (for example, you need IELTS/TOEFL for most universities in English-speaking countries). If your language exam results are not up to their standard, you might be admitted to a foundation coutse before actually starting university.

Lastly, ensure you have sufficient funds to support yourself throughout university.
