It is best that before you sit for any exam, you decide which country you want to study medicine in. This is because the admission requirements often vary with country. For most countries, you can gain entry to a bachelor's program with just your 12th standard score or an SAT score.
On the other hand, for undergraduate admission to a program in medicine in the USA, you will have to give the MCAT (Medical College Admission Test). Similarly, for admission in the UK, you will have to give UKCAT (UK Clinical Aptitude Test). Therefore, first decide your country of study before sitting for any examination.
It is best to also decide the country where you want to pursue a career in medicine in. This is because some countries may not recognized medical qualifications awarded in other countries. For example, US institutions often don't recognize most Indian-awarded medical degrees (except MBBS), and this may lead to you repeating qualifications, or going through the tiresome process of accreditation.
Apart from this, you will need to sit for an English language proficiency test as well for ANY country of application. This includes tests like IELTS Academic and TOEFL iBT.
