Yes, you're eligible to write IIT JAM after clearing 12th in medical stream if you have scored 55 percent marks or 5.5 out of 10 aggregate in languages and subsidiaries of all the years combined and you must possess a Bachelor's degree. For more details, click on IIT JAM Eligibility Criteria

JAM is an annual entrance exam conducted by IIT as part of the admission to the various Postgraduate programmes offered by the participating IITs. The eligibility criteria of JAM 2019 will be different for the different courses offered by the participating IITs.
Since you +2 in physics chemistry and biology you are eligible for MSc biotechnology.You can do undergraduate degree in any branch or subject to be eligible for MSc biotech.

I was not able to understand what you meant? Have you meant about entry to IIT Kanpur after Class 12 through IIT JAM?
If yes, JAM is Joint Admission test for M.Sc. This is meant for admission to various IITs for M.Sc. in Chemistry, Biotechnology, Physics, Geology etc. The eligibility is Bachelors degree. So with Class 12 eligibility alone you are not able to apply for JAM.
