Filling out an application to get a seat in government hostel for ug course b pharmacy in Bagalkot, Karnataka 2020-21. The form is rather complicated consisting of five pages. On the first page a student is required to provide their address, name, place and date of birth. Next an applicant is supposed to provide relevant information in the following sections:

• Academic record

• Language proficiency level

• German language proficiency

• Business courses

• Work experience

The form must also be signed and dated.
