How to make best revision notes for preparation for jee exams, this answer is inside you. You only know your weak and strong points / spots. So no one can tell that how you should make notes. Because it is on student whether he/she will be comfortable with that method or not. Because every person's mind is different and works differently. So the best way to make a revision notes for preparing exams to ask yourself.
Now still as you have passed to a query then I am telling you some short preparation strategies of the revision notes that I had done in my time when I was a JEE advanced aspirant.
First go through your entire notes or revise thoroughly the notes 1 time and don't try to make revision or formula chart after revision for just one time. Now revise your notes the second time, now after revising second time you will get refined with concepts and after that start preparing revision notes or formula charts simultaneously by revising notes third time. And if possible then try to make diagrams too.
For making revision notes or charts, take loose sheets and write only the formula and important point and how to apply formulas, sequence wise and in a proper order of a particular chapter.
In Physics you have to apply the concepts and have to remember the formula so do the same as told in above line.
Now in physical chemistry write down the formulas and its concept also with examples. And just practice question. If you facevany new problem or new concept which you haven't seen before, then write that formula or concept in your revision notes. For Organic Chemistry, there is no particular advice for making formula chart for revision notes because organic chemistry is a complete branch of chemistry which needs revising each and every time. And you cannot separate any chapters because every chapters is correlated with each other. First of all read the concepts and theorems everything, that is read all the theory part first in a chapter or topic. Mark all the important points , theorems, formulas and concepts. Mark everything that you feel you might not be able to remember.

For chemistry write all the compounds name nas formulas , make a different chart and tables. Don't just simply copy statements. Try to write them in your own words and in short.
For mathematics also write the theorems and formulas for better understanding and remembering.
