The different institutions providing this quota furnish this concession certificate on their official website. You have to fill out this form and submit it to the relevant official department.
There are different procedures for different states. I am providing the procedure for UP state. You have to apply offline for this
Desirable applicants who insist on educational concessions in govt acknowledged public/private universities or central/state govt benefited universities have to apply at the AG (Adjutant General ) Dept., Army HQ.
A compensation request document should be furnished by having the applicant's name and service digit of any one of the parents of aspirants with the name and class of the aspirant.
Acquire the Contingent Bill from your college provided as in Pg. No.5 and 6 of the Education Concession Program

You have to read all the instructions furnished in the link before you fill out the form.
Then fill in all the conditions expected in that Bill.
Then you have to assemble the revenue stamp, and your parent has to sign over the stamp.
You have to make the document signed by the Head of the college.
Other documents you need are
Xerox copy of all documents and receipts.
If operating staff, the papers must be fulfilled through the unit.
If already resigned staff, the documents must be delivered by mail to the AG’s Branch, Army HQ.
After processing the request, the prices will be assigned to your bank account.
Submission of these documents is necessary.
Actual sheets of uniform, study materials, schooling fee, transportation fare, and hostel payments must have been combined with the statement.
