You can apply for pwd certificate in a medical hospital. The certificate is issued by an authority which is usually the Chief Medical Officer (CMO) or Sub Divisional Medical Officer (SDMO). The date when the certificate is issued will be mentioned on it. The certificate will be valid for 5 years after that.
