Hi aspirant.
You can download the performas from:
www.wbmcc.nic.in
You need to fill up performa A1 and get it signed by your school principal. Also fill in performa A2 and get it signed by a gazetted officer of West Bengal government. These stand as proof that you are from West Bengal domicile.
