Hello
The official format for medical certificate will be available in the official website. You have to download that and get it printed. Then , you have to visit any government hospital or government clinic and the doctor will do the necessary check ups and fill the certificate that you had downloaded. That stamped document is to be submitted as the medical report.

If I visited private doctor than there is problem or not. Please tell me

You need to go to govt district hospital as the certificate requires the stamp of CMO which is not possible through private hospital.
