I am a private candidate(CBSE), and I don't know what to fill in the name of institution and address of institution in the admission form for college?

Since you're a private cbse candidate, you can fill the name of your previous school in the place of name of institution, and in the place of address, here also you can fill the address of your previous school, by previous school it does mean to fill the name and address of the school from where you have passed 11th std.
