i am an engineering graduate (12 pcm) but now i want to pursue mbbs .Does i have to repeat 11 12 with (PCB)

No, you don't need to do 11th again.
Open board students are eligible for neet 2021 so what you can do is directly go for NIOS on demand 12th examination with 5 subjects including physics chemistry biology and English and for the 5th subject you can choose any subject of your choice and then after qualifying it you will be eligible for Neet examination provided you must get :-

at least 50 % in aggregate of PCB i.e Physics, Chemistry and Biology/Biotechnology if belongs to general category

at least 40% in aggregate of PCB if belongs to st/Obc/ Sc
at least 45 % in aggregate of PCB in case of pwd candidates to be eligible.
If you manage to fulfil this% requirement then you will be eligible for neet and after qualifying neet you can get into mbbs, bds, ayush courses etc as well.
Also,
* to be eligible for neet 2021 one has to be at least 17 years old or more by 31st December 2021
* There is no upper age limit for Neet 2021
* There's no limit for the maximum number of attempts/times one can appear in NEET.
