Well if you are looking for government Medical college in KERALA in your chances are quite low because this year cutoff goes higher than last year
But still as you are belonging from obc category candidates then you can try but 489 marks are low .
Although at your marks you can try for the government BDS seat in best college.
Also other Medical courses too.
For getting mbbs in government college you must need to score atleast 540+ marks ( estimated figure only)
For good government college.
But still you can try for 2nd or further rounds you may get chance.

As you mentioned with OBC category and 489 marks, your chances of getting a decent government college in your state through state quota(85%) are negligible.
However you might get some good private medical college in your state through state quota.
As the admission to medical colleges based on NEET is given unsed All India Quota(15%) and state quota(85%).
However cutoffs change every year depending upon level of toughness of exam. So hope for the best and follow the counselling process.
