Yes Income certificate is a necessary document that is needed during the counselling stage. Income certificate is issued by the state government as an evidence of confirming your annual income. As per the required documents, I don't think so that you can show income declaration form instead of income certificate. But for full clarity on this you should mail or contact A.P state council of higher education.

The validity of income certificate is one financial year.

For eg, if considering the financial year of 2018-2019, the validity of the income certificate is from 1st April 2018 to 31st March 2019. Since your income declaration form is dated on 29th September 2016, this is way too far from validity dates. It's better to get a new income certificate from the concerned income certificate-issuing bodies.

It is necessary to produce income certificate for the online application. In addition to that, there are several documents are requested to upload at the time of online application.

Another important thing is that Candidates Adhaar card number is mandatory.
