i am mbbs 1st year student can i give class 12 board exam privatly with maths as additional or main subject??

Yes definitely you can give class 12th board examination again for any of the subject in which you want to improve your marks, through open boards such as NIOS or any other state board. These boards conduct On Demand examinations through which one can opt only to appear for single or any number of subjects in which he/she wants to improve the marks. So even if you are in MBBS 1st year then also you can appear for it.

See if you wish to add a subject in your 10+2, then you can apply for the On demand examination in NIOS. Through this, you can attempt only the paper of Mathematics through NIOS & you can use the marksheet along with your boards marksheet, which will be accepted by any Government Recognized institute for admission.
