No dear, your application not rejected. You can edit your form. Any examination like JEE Mains or NEET can give another option for candidates for changing their form or any mistake happen then change it and edit form.
So this year also NEET 2021 correction window open. The date of correction window is 8th August 2021 and it's available till 12th August 2021. If extension occur then NTA will inform it. So you can give your photo in that time. Till now don't take any pressure just prepare yourself for NEET examination.

As you mentioned that you haven't uploaded your photograph with name and date, so let me tell you it's not necessary to upload to your photograph with name and date and it won't be rejected so don't worry.
Even if it is required to do so you will be intimated by sms or mail and you will get time to edit or correct your mistakes in the form. So, you need not worry your application won't be rejected.

You don't need to worry even if you have not mentioned date and your name and your photo ,it should not be the reason for the rejection of your application form.
This is because,
It is not mandatory but just preferable, to mention your name and date of photo on your photographs
The page number 31 of neet 2021 information brochure says that your photographs must be taken on or after 01.01.2021 and preferably mentioning the name of the student and the date on which the photograph was clicked - post card photo and passport size photo.

Also, in case of any discrepancies in your application form NTA mails you on your registered Email id.

You can correct your photograph using correction window and upload a photograph with a name and date.
Neet correction window will open on 8th August and the last date to use it is 12th August.
Here's how you can use it to change the uploaded images :-
* visit neet.nta.nic.in
* login using your application number and password
* An option to change the images will be there on your screen, click on it
*Now you can change your uploaded images
*Then you will get an otp on your registered mobile number which you will have to enter in order to confirm your correction.

Specifications for photo:-
-----) your passport size photograph,(size: 10 kb to 200 kb)
-----) your postcard size photograph (4”X6”) (Size: 10 kb to 200 kb)
your photographs must be taken on or after 01.01.2021 and preferably mentioning your name and the date on which the photograph was clicked
you shouldn't be wearing any cap or goggle,
spectacles are allowed only if you use them regularly.
at least 80% face coverage should be there in your photographs
your ears should be visible
Should be coloured one
the background should be white.
Should be in jpg format
