I forgot to mention surname in neet application.what should I do now. It creates problem at the time of exam.

Sorry to say but the correction window is already closed. Yes, it will create problem first at your exam centre and then during CounsellingS. Carry all your official documents belonging to you to prove that the person in application form is you at exam centre. They might allow you to give but during counselling it’s much more strict and cannot be predicted.

If I make affidavit than it will be helpful to prove that person in application form is same as other documents.

Yes you can but the acceptance tests on the officials and that cannot be said.
