If we consider previous year cutoffs of government medical colleges for the candidates of your category then you do have chances of getting a seat. Last year collges do had cutoff less than 40-50 marks than your score. This year the cutoff may increase but still you have chance. you can also get a seat in private colleges. As you mentioned with BC B category and 484 marks, your chances of getting a decent government college in your state through state quota(85%) are quite high.
As the admission to medical colleges based on NEET is given unsed All India Quota(15%) and state quota(85%).
